End of life planning
Death is not something we like to think about, but it is an inescapable aspect of life. It is something important to plan for, and write a conclusion to your story on your own terms.
If ever a day comes when you cannot respond, who would you like to speak for you? What would your wishes be, based on your individual values and beliefs? Would you want your healthcare providers to use aggressive CPR and life support in an effort to bring you back, or would you prefer a natural death?
Advanced care planning involves a process of discussion between the patient, family, and health care provider, to provide education on the diagnosis and prognosis, decide on goals of care, and help the patient find hope and meaning in life, with a sense of spiritual peace.
